Beyond Discharge: Supporting Young People at High-Risk for DKA through a Multidisciplinary Youth and Parent Group- (POP-UP 2024)

This presentation will provide attendees with an in-depth look into the design, implementation, and evaluation of a multidisciplinary support group for high-risk youth with type 1 diabetes; including how the group transitioned successfully to and from virtual during the global pandemic. Developmental considerations and patient and family-centered care will be highlighted in addition to learning opportunities and next steps. Participants will gain an understanding of a working model that can be replicated by child life specialists and the multidisciplinary teams they work with for many different chronic illnesses and conditions.

- Participants will be able to define goals for a community-based intervention for youth and caregivers of those with T1DM

- Participants will be able to describe a robust interdisciplinary intervention to address the psychological, social, and educational needs of parents and youth    with T1 diabetes

- Participants will be able to discuss changes to sustain a new program for diabetes youth and families with the onset and restrictions of a pandemic

- Participants will be able to review and evaluate outcomes presented for youth and families managing T1DM

With a Master of Science degree from the University of Georgia, Shelley McDaniel is currently a Certified Child Life Specialist (CCLS) on the south side of Chicago at UChicago Medicine. Shelley began working with children and families with Type 1 diabetes over a decade ago, promoting community and connection at a summer camp for children and youth with Type 1 diabetes. In the present day, Shelley supports children with Type 1 diabetes both in and out of the hospital through a multidisciplinary diabetes youth and parent group. Shelley continues to seek ways to promote coping, resilience, and positive adjustment for patients and their families through developmentally appropriate interventions, education, and advocacy.
